La Toya Darden has worked in community health care for the last 10 years. La Toya Darden has directed Houston Medi-Dent Mobile Health, Inc.  As Program Director, she demonstrated abilities in internal and external marketing, financial, management and dental software technology. Wrote successful grants for children’s dental program Analyzed and wrote appropriate responses to requests for proposal (RFP), requests for quote (RFQ) and requests for information (RFI) Set up and established new territory by devising a technique for introducing the concept of oral preventative services for underserved school-age children Designed an introductory “Partnering with the Community”, “Miles of Healthy Smiles” program that is now standard for the agency.
Served as programs administer and grant writer for program.
Developed complete managerial outline for continued growth and success of agency Established a professional relationship and maintained communication with individual school personnel in targeted school districts. Promoted and facilitated dental prevention education programs in identified schools utilizing various marketing and promotional strategies. Maintained and sustained strong relationships between donors and the non-profit agency in return provided dental services to thousands of undeserved school children.
Under her direction as Chief Operations Officer for Central Care CHC, La Toya Darden has implemented HIV primary care and collegiate inoculation medical programs for underserved patients residing in South Central Houston and through out Houston, Texas.
She has successfully orchestrated funding through Houston Endowment for renovations for Central Care CHC main site and secured contracts from HISD for the Convenience Clinic Benefits Plan to provide Primary Preventative health care to HISD employees. In 2011 La Toya Darden was a recipient of the Johnson and Johnson Execute Healthcare Fellowship Program.
The UCLA/Johnson & Johnson Health Care Executive Program is a management development program exclusively for executive directors and leaders of health centers and of other community-based health care organizations.  The program provides extensive management and leadership development at the UCLA Anderson School of Management in Los Angeles, California. Participants engage in a rigorous but relevant curriculum that provides the requisite skills, knowledge and abilities to successfully manage and lead their organizations in a changed world for healthcare delivery. The program has graduated nearly 600 participants since its inception in 2002.
 La Toya Darden received her B.A in Psychology from Texas Southern University in Houston, Texas.

Dr. Altaf Ibrahim (Chief Operations Officer)

Dr. Altaf Ibrahim (Chief Operations Officer)

Dr Altaf Ibrahim is a senior healthcare executive whose career goals incorporate the acquisitions of knowledge and skill sets that will help alleviate the sufferings of human populations. 
He has extensive health care experience and has served among other positions, as the Corporate lead for the National Clinical Criteria Program for Blue Cross Blue Shield of Michigan, Corporate Director of Quality and Accreditation for YVWFC, the second largest community based health care system in the US, based in Washington and Oregon, as the Regional Director of Quality Resource Management for three Christus Health Hospitals in Louisiana, and as the Senior Director of Operations, and Projects for Legacy Community Health Services in Texas.

He practices transformational leadership, and believes in mentoring associates.  Dr Ibrahim sees himself, as a change agent, a facilitator, whose passion is to oversee a lean, high quality, operations department, where team members are treated with dignity and respect. 

As he states, it is people who make the difference between success and failure of any enterprise.  No computer can substitute for the human spirit. 
We have to work for the preservation of that spirit!

Dr. Ujjaval Jariwala (CMO)

Dr. Ujjaval Jariwala (CMO)

Dr. Jariwala is originally from India. He was raised and went to medical school in Surat, a rapidly growing metropolitan city in the western part of India. Subsequently, he moved to Baltimore, Maryland to complete his Internal Medicine Residency training at Greater Baltimore Medical Center.
Dr. Jariwala believes it is a privilege to be able to positively affect the health of the patients not only by treating their current disease but also prevent future illness. The ability to establish a close physician-patient partnership and improve the overall health of his  patients is what he loves about practicing medicine. Dr. Jariwala describes his style as a Medical Doctor in terms that he likes to listen carefully to his patients.

Knowing about his patient’s background and what they do for work, helps him to understand their problems better. His personal motto is “The secret of the care of the patient is in caring for the patient” – Dr. Francis Peabody. Dr. Jariwala overall goal is to help patients navigate the complex health care system. He is very passionate about finding ways in which new technology can make it easier for patients to manage their medical problems.
A few exciting things about Dr. Jariwala is that he loves going on long hikes.

Also, he is a true foodie.  “Nothing more special to me than homemade Indian food” -  Dr. Jariwala.
